SHOT – serious hazards of transfusion – is the UK national haemovigilance scheme. All UK hospitals  report their adverse incidents related to blood transfusion (about 3500 per annum) encompassing surveillance from donor to recipient.  The annual report will be launched at the SHOT symposium on July 12th. This one-day meeting will focus on the incidence and causes of adverse incidents related to transfusion, including errors due to human factors, which are the majority.
